Patients Forms:
Call the office to schedule, we will send you a link to access your intake forms to complete online. If for some reason you are unable to complete these online, alternatively, you may download a hard copy of the intake form, (see New Patient Form in box below), complete and bring with you at the time of the visit.

Cancellations:
As a courtesy, we request that you give 24 hrs. notice for any cancellations. We invest time preparing for your visit and schedule plenty of time for an unhurried visit.
No shows or cancellations less than 24 hours will be charged the full price of your appointment.
In order to create a peaceful and restful atmosphere we ask phones be silent during your visit.
Insurance Information:
Private Insurance:If you have insurance our office does provide courtesy insurance billing. However we are out-of-network with all insurance companies. We are only able to provide billing for insurance companies that have out-of-network acupuncture coverage.
Deductibles and copayments are due at time of service. Any balances after insurance payment will be your responsibility
Medicare:
We are not Medicare providers. Our medicare patients pay in full at time of service.
If you have a secondary insurance that does have acupuncture coverage we will be happy to provide with paperwork to submit for reimbursement from your secondary insurance company.
While we are happy to be of help to our patients with insurance benefits, we cannot guarantee payment.
We recommend you call your insurance company prior to scheduling your appointment to verify your acupuncture benefits.
Payment to Massage therapists:
Massage therapists are paid directly. Cash, check and/or Venmo are acceptable forms of payment.
