We appreciate patients being mindful of voice volume, so as a courtesy to our patients, we request that you turn your phone to silent during your visit. We also ask that when possible you refrain from wearing perfume or cologne, as we have some patients sensitive to the scent.

CANCELLATIONS

If you need to cancel or reschedule your appointment, please give us at least 24 hours notice. Less than 24 hour notice there will be a $25.00 cancellation fee. “No Show” appointments will be charged the full amount of the visit.

ASSIGNMENT OF BENEFITS ( FOR PATIENTS WITH INSURANCE)

If you carry a private insurance you will be expected to assign benefits (tell your insurance company to forward insurance payments directly to the Acupuncture Center of Santa Maria for services provided here.) We urge you to carefully review your insurance coverage and plan prior to your visit. Policies are confusing, misleading and rarely pay everything. Please be aware that coverage varies with out of network providers.

If you are an insurance patient, our office provides insurance billing. Please provide us all necessary insurance information so we can bill your insurance provider. Please check with your insurance benefits before scheduling by calling the number provided on the back of your card. Payment in full is required until your insurance deductible has been satisfied.

Insurance benefits are a matter between the patient (the insured) and their insurance company.
Although we do provide insurance billing from our office, if your insurance company fails to pay on your claims you will be responsible for the full cost of service. Once we verify coverage, we will review benefits with you. Although you may have insurance coverage, please note that insurers state that: “Benefits quoted are not a guarantee of payment. Payment will be determined by the insurance company using all portions of the member’s plan when a claim is received."

SPECIAL CASES

We are an In Network Provider for PPO Blue Shield Plans.

If you have workmans comp or have sustained a personal injury (either work related or automobile) and are working with an attorney, we take these on a case by case basis. Please let the office staff know before you schedule your appointment.

MEDICARE EXCLUSIONS

Medicare does not cover Acupuncture Services and those with Medicare are responsible to pay their balance in full at the time of service.

If you have a secondary insurance carrier that will pay for Acupuncture benefits when Medicare does not, we will gladly give you the needed documentation to bill your secondary insurance directly.
